Make a Referral
Referring someone for live-in care is straightforward. We welcome referrals from families, GPs, social workers, district nurses, discharge teams, and other care professionals.
Who can make a referral?
Anyone can refer someone to Better Hearts — whether you are a family member, a friend, or a healthcare or social care professional. We treat every referral with the same care, urgency, and confidentiality.
If you are a professional making a referral on behalf of a patient or service user, please use the form below or call us directly. We aim to respond to all referrals within one working day.
